This is our Standard Service Agreement that ALL non-commercial customers must sign to activate service. In order to activate service you will need to complete and return this agreement along with a copy of a state issued photo ID to the Administrative office and pay the deposit and fees to activate service. Contact one of our Customer Service Representatives for exact deposit/fee amount.

There is a flat $50 fee. For a non-refundable fee, a 72-hour real estate turn on is available on Monday through Friday. Fee must be paid before service is activated. Service is only offered to a license Real Estate Agent or a licensed Inspector. Property is required to have the customer cut off valve before water can be turned on for this service. Excessive water usage outside normal inspection may be charged back to the service Agent/Inspector.

Customers who want to change their mailing address, phone number, or other account information will need to complete this form and return it to the Administrative Office. ONLY the account holder (name on account) is allowed to make any changes to the account.
Customers can request that personal information contained in our utility records not be released to unauthorized persons. Fill out this form and turn it into the Administrative Office, with a one-time $5 payment for the fee to cover the cost of implementation.

This is the ACH Debit form for customers who want to have their water bills debited from their bank account on a monthly basis automatically. this service is our in-house free service to our customers. Customers will still receive their water bill monthly for informational purposes but it should state that it was drafted on the billing. Complete the form and return it to our Administrative Office with a voided check. (Note: There are online options to set up reoccuring ach and credit card payments that have a fee. This is the free service done in-house)

DISCLOSURE OF PERSONAL INFORMATION FORM
Chapter 182, Subchapter B of the Texas Utilities Code makes confidential a water utility customer’s address, telephone number, account records, social security number,[1] and information relating to the volume or units of utility usage, or the amounts billed to or collected from the individual for utility usage. However, utility customers may elect to authorize disclosure of this information by completing the disclose form.

- Any person(s) who are wanting to set up new Commercial accounts for new built commercial buildings, major updates to commercial buildings, and/or demolish/rebuild commercial buildings will need to complete the Engineer Utility Review process.
- Any person(s wanting to create a subdivision, multi housing, and/or RV park are considered non-standard and will need to complete the Engineer Utility Review Process.
- Meet with the General Manager with plans.
- Complete the Engineer Utility Review Form
- Pay the required Engineer fee of $1,000. This fee is used for any Engineer costs for review of the new service. If any funds are left over they will be mailed to the customer. If any funds are due then they will be billed.
- Submit plot maps, building details, etc.
If you are wanting to close your account and have service deactivated, fill out this form and file it with our Administrative Office. We will turn off the water service. Please be sure and give a good forwarding address to mail the final bill or refund check to.
This form must be completed when paying for new water/wastewater taps or relocation of grinder tanks and/or lines.

If you would like to request public information form the District, fill out this form and return it to the Administrative Office. You may also submit a written request to the office by fax, mail, and/or in person.
Please note that Public Information Request can take up to ten (10) business days to gather the information requested. Be sure that you put a phone number that you can be reached in case there is need for clarification on the request and also so that you can be contacted about the costs of the request.
